After his performance he gave his jacket to a young man named Jake.
Jake was born with Lesch-Nyhan syndrome.
It is a rare metabolic disorder that occurs before birth, mostly in boys.
In the video that has since gone viral, Jake is beyond excited when Nelly gives him his jacket and hugs him.
